New York state requires that
parental permission forms are signed and turned in before the start
of each sport season. So this process must be repeated for the fall,
winter and spring sports seasons. The state also requires that
all students participating in interscholastic athletics have a physical
once every calendar year (this can be performed by the family's own
private physician or the school's physician).
You will need to complete and return the forms
available at the links below (HE 104, HE
106A, HE 106B). It is your responsibility to attend one of the
Updates for Interscholastic Sports or you
cannot play a sport. For more information, call
